Let’s start over.
For the record I first went to Dr. Ellis in August, 2012. I thought it was June 2012 but it was August 2012. I went to her for a second opinion for what was diagnosed at the time by a previous doctor and confirmed by a previous neurologist as Ocular Migraine. Upon researching Ocular Migraine I was certain the diagnosis was incorrect. Bold text indicates doctors notes in my medical records.
My symptoms were ‘an off-balance sensation… with predominant visual symptom left upper visual field loss, leaning to the left‘.
The medical records from my previous doctor that were sent to Dr. Ellis were missing when I requested my medical records from Dr. Ellis. I was told by Dr Ellis’ staff that my previous medical records amounted to only one page and they were not required to include that one page as part of my medical record. Upon receiving this information, I reached out to Stoneview Internal Medicine to confirm. They claimed to have submitted all of my medical records in full to Dr. Ellis.
Dr. Ellis referred me to Dr. Kistler. He confirmed ‘she notes a flickering sensation in the left visual field at times‘.
My follow up with Dr. Ellis was in September of 2012. She states that I was placed on Relpax. I never took Relpax. I was given a sample of something at the time to take for migraines but since I never had a migraine, I never took it. Nor did I ever fill that prescription.
Dr. Ellis’ notes for my second appointment state F/U to discuss lab results and Neurology Consult‘. This appointment is the appointment where we were continuing to find a cause for the symptoms that Papilledema presented. She states ‘the patient is a 47 year old…who presents with her migraine headaches. Wrong! No I did not have migraine headaches. She was floundering around from the very start. Do I have a headache now, yes. But not then. Dr. Ellis got it wrong from the second appointment! And this misguided her to continually mistreat me for years while I was under her care. I must rest right now but there is so much more to come.
